Ремоделирование сердца представляет собой сложный двунаправленный процесс, при котором структурные и функциональные параметры сердца отклоняются от нормы в ответ на нарушение внутрисердечной гемодинамики после острого инфаркта миокарда. Обратное ремоделирование левого желудочка (ЛЖ) проявляется улучшением систолической и диастолической функции, за счёт уменьшения объёмов полости сердца. Цель исследования – провести анализ динамики и степени обратного ремоделирования ЛЖ после хирургической коррекции постинфарктных аневризм левого желудочка. На основании результатов разработать модель прогнозирования ультразвуковых параметров у пациентов в послеоперационном периоде.</p></sec><sec><title>Методы</title><p>Методы. Проведен ретроспективный анализ результатов лечения 174 пациентов, которым выполнялась пластика постинфарктной аневризмы, с последующим аортокоронарным шунтированием (АКШ) в условиях ГБУЗ НО «НИИ – СККБ им. акад. Б.А. Королёва» г. Нижний Новгород в период с 2011 года по 2022 года. Пациенты распределены на две группы. В первой группе выполнялась пластика по Кули и АКШ, во второй пластика по Дору и АКШ. У пациентов имелись различные факторы риска, которые влияли на формирование процессов ремоделирования ЛЖ. С помощью ультразвукового исследования получены данные ЛЖ и других структур сердца. Благодаря искусственному интеллекту выполнено прогнозирование возможного объёма оперативной реконструкции сердца.</p></sec><sec><title>Результаты</title><p>Результаты. Получено достоверное снижение ультразвуковых показателей объёма ЛЖ у пациентов в различные сроки после операции. Наиболее значимым прикладным итогом исследования стала разработка клинически применимой модели машинного обучения для прогнозирования результатов операции. Её высокая точность (подтверждённая низкой медианной ошибкой) позволяет использовать модель для предоперационного планирования с целью индивидуализации хирургической тактики. Модель помогает определить «золотую середину» в объёме резекции — достаточную для запуска обратного ремоделирования, но безопасную с точки зрения риска синдрома малого выброса. Внедрение подобных систем поддержки решений на основе искусственного интеллекта напрямую способствует повышению безопасности операций и улучшению исходов для пациентов.</p></sec><sec><title>Заключение</title><p>Заключение. В кардиохирургии ключевым показателем успеха является обратное ремоделирование ЛЖ на фоне ремиссии СН после операции. В последние годы оценка этого процесса стала краеугольным камнем клинической практики, поскольку именно он служит главным предиктором благоприятного долгосрочного прогноза для пациентов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Cardiac remodeling is a complex bidirectional process in which structural and functional parameters of the heart deviate from the norm in response to impaired intracardiac hemodynamics following acute myocardial infarction. Reverse left ventricular (LV) remodeling is manifested by improved systolic and diastolic function due to a decrease in cardiac chamber volume.</p></sec><sec><title>Objectives</title><p>Objectives. To analyze the dynamics and degree of reverse remodeling of the left ventricle after surgical correction of post-infarction aneurysms of the left ventricle. Based on the results, develop a model for predicting ultrasound parameters in patients in the postoperative period.</p></sec><sec><title>Methods</title><p>Methods. A retrospective analysis of treatment outcomes was conducted for 174 patients who underwent post-infarction aneurysm repair followed by coronary artery bypass grafting (CABG) at the Nizhny Novgorod Research Institute of Cardiology and the Nizhny Novgorod Clinical Hospital named after Academician B.A. Korolev between 2011 and 2022. Patients were divided into two groups. The first group underwent Cooley repair and CABG, while the second group underwent Dor repair and CABG. The patients had various risk factors that influenced the development of LV remodeling processes. Ultrasound imaging was used to obtain data on the LV and other cardiac structures. Artificial intelligence was used to predict the potential extent of cardiac reconstruction.</p></sec><sec><title>Results</title><p>Results. A significant reduction in ultrasound measurements of LV volume was observed in patients at various postoperative times. The most significant practical outcome of the study was the development of a clinically applicable machine learning model for predicting surgical outcomes. Its high accuracy (confirmed by a low median error) allows the model to be used for preoperative planning to individualize surgical tactics. The model helps determine the "sweet spot" in the extent of resection — one that is sufficient to initiate reverse remodeling but safe from the risk of low-output syndrome. The implementation of such AI-based decision support systems directly contributes to improved surgical safety and patient outcomes.</p></sec><sec><title>Conclusion</title><p>Conclusion. In cardiac surgery, a key indicator of success is reverse LV remodeling against the background of heart failure remission after surgery.
