Изучить эффективность и безопасность расширенной криобаллонной аблации у пациентов с персистирующей и длительно персистирующей фор­мами фибрилляции предсердий в сравнении с классической методикой кри­обаллонной аблации.</p></sec><sec><title>Методы</title><p>Методы. В исследование включено 117 пациентов, с персистирующей (73 па­циента) и длительно персистирующей (44 пациента) формами фибрилляции предсердий, медиана фракции выброса левого желудочка составила 58,0% [54,0; 60,0]. Медиана размера левого предсердия составила 4,2 см [4.00; 4.60]. Выполнен сравнительный анализ результатов классической криобал­лонной аблации – группа сравнения (56 пациентов) и криобаллонной аблации дополненной изоляцией задней стенки левого предсердия – группа наблюде­ния (61 пациент).</p></sec><sec><title>Результаты</title><p>Результаты. Эффективность криобаллонной аблации дополненной изоляци­ей задней стенки левого предсердия составила 73,8% (n – 45), криобаллонная аблация 67,9% (n – 38) при 12-месячном сроке наблюдения. Различия без­рецидивной выживаемости, оцененные с помощью теста отношения правдо­подобия, были статистически значимы (p=0,030). Статистически значимых различий в безопасности выявлено не было. В ходе данного исследования не было зарегистрировано жизнеугрожающих осложнений.</p></sec><sec><title>Заключение</title><p>Заключение. Обе методики показали сопоставимо безопасные результаты. Изоляция задней стенки левого предсердия является более эффективным методом катетерного лечения персистирующей и длительно персистирую­щей форм фибрилляции предсердий в сравнении с классической изоляцией устьев лёгочных вен.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To evaluate the efficacy and safety of posterior wall isolation using the cryoballoon ablation in patients with persistent and long-standing persistent forms of atrial fibrillation in comparison with pulmonary vein antrum cryoballoon isolation.</p></sec><sec><title>Methods</title><p>Methods. One hundred seventeen (n=117) patients with persistent (n=73) and long-standing persistent (n=44) atrial fibrillation, median left ventricular ejection fraction was 58,0% [54,0; 60,0]. The median size of the left atrial was 4,2 cm [4,00; 4,60]. A comparative analysis of the results of cryoballoon isolation supplemented with isolation of the posterior wall of the left atrium – group A (61 patients) and classic cryoballoon pulmonary vein antrum cryoballoon isolation – group B (56 patients) was performed.</p></sec><sec><title>Results</title><p>Results. In one-year follow-up the efficacy of cryoballoon isolation supplemented with isolation of the posterior wall of the left atrium was significantly higher – 73,8% (n=45) than in cryoballoon pulmonary vein isolation – 67,9% (n=38) (p=0,030). There were no statistically significant differences in complication rates between the groups. No life-threatening complications were recorded during this study.</p></sec><sec><title>Conclusion</title><p>Conclusion. Both methods showed comparable safe results.
