Оценить эффективность проведения катетерной аблации (КА) фибрилляции предсердий (ФП) у пациентов с систолической дисфункцией левого желудочка и влияние вмешательства на тече­ние хронической сердечной недостаточности (ХСН) по сравнению с применением антиаритмической терапии (ААТ).</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование включены пациенты (n=80) с фракцией выброса левого желудочка (ФВ ЛЖ) менее 50% и ФП, которые были последовательно распределены на 2 группы: 1-ой группе (n=40) выполнена операция КА, 2-ую группу (сравнения) со­ставили пациенты на ААТ. Исходно и через 6 месяцев наблюдения выполнены трансторакальная эхокардиография, определение кон­центрации NT-proBNP, тест 6-минутной ходьбы и оценка качества жизни по данным Миннесотского опросника.</p></sec><sec><title>Результаты</title><p>Результаты. Через 6 месяцев наблюдения отсутствие документиро­ванных пароксизмов ФП наблюдалось у 80% в группе КА против 45% в группе антиаритмической терапии (p=0,001). В динамике в группах КА и ААТ выявлен значимый прирост ФВ ЛЖ, однако в группе срав­нения он был меньшим (57% [50,5; 60] в группе КА против 46% [40; 50] в группе ААТ (p=0,0001)). Получено снижение КСР левого желу­дочка и размеров и объемов левого предсердия в группе КА, тогда как в группе ААТ динамики данных показателей в течение полугода не произошло (p=0,0001). На фоне восстановления и удержания си­нусового ритма наблюдалось улучшение клинического течения ХСН: в группах КА и ААТ через 6 месяцев уровень NT-proBNP составлял 196,6 [116,5; 327,1] и 540,5 [382,8; 861,2] пг/мл (0,0001), дистанция теста 6-минутной ходьбы увеличилась до 470 [400; 500] и 368 [330; 400] м (р=0,0001), соответственно, снижение ФК до 1,15±0,67 и 2,05±0,55 (р=0,0001) и улучшение качества жизни (р=0,0001). </p></sec><sec><title>Заключение</title><p>Заключение. КА является эффективным методом лечения ФП у па­циентов с ХСН. Применение КА ФП у пациентов с ХСН способствует обратному ремоделированию камер сердца, снижению уровня NT- proBNP, а также улучшению качества жизни и повышению толе­рантности к физической нагрузке.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To evaluate the efficacy of catheter ablation (CA) of atrial fibrillation (AF) in patients with left ventricular systolic dysfunction and the effect of the procedure on the course of chronic heart failure (CHF) compared with the use of antiarrhythmic therapy (AAT).</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study included patients (n=80) with left ventricular ejection fraction (LVEF) less than 50% and AF, who were divided into 2 groups: the first group (n=40) underwent CA, the second group included patients on AAT. At baseline and after 6 months of follow-up, transthoracic echocardiography, determination of NT-proBNP concentration, 6-minute walk test and The Minnesota Living With Heart Failure Questionnaire were performed.</p></sec><sec><title>Results</title><p>Results. After 6 months of follow-up, the absence of documented AF paroxysms was observed in 80% in the CA group versus 45% in the AAT group (p=0,001). A significant increase in LV EF was revealed in both groups, however, in CA group it was more significant (57% [50,5; 60] in the CA group versus 46% [40; 50] in the AAT group (p=0,0001)). There was a decrease in the left ventricle ESD and the left atrial size and volume in the CA group, while in the AAT group the dynamics of these parameters did not change within six months (p=0.0001). An improvement in the clinical course of CHF was observed in both groups: the level of NT-proBNP was 196,6 [116,5; 327,1] pg/ml and 540.5 [382,8; 861,2] pg/ml (0,0001), the 6-minute walk distance increased to 470 m [400; 500] and 368 [330; 400] m (p=0,0001), respectively, the improvement of the functional class (1,15±0,67 and 2,05±0,55 (p=0,0001)) and the quality of life (p=0,0001) were observed.</p></sec><sec><title>Conclusion</title><p>Conclusion. CA is an effective method for patients with AF and CHF.
