Оценить эффективность аблации очагов роторной активности, дополненной изоляцией легочных вен у пациентов с персистирующей формой фибрилляции предсердий в снижении частоты рецидивов после вмешательства в сравнении с классической методикой антральной изоляции легочных вен.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование было включено 86 пациентов, которые были поделены на группу А – антральной изоляции легочных вен и группу В, в которой изоляция легочных вен была расширена абляцией роторных областей в левом предсердии. В группе В непосредственно  перед аблацией проводилось неинвазивное картирование  роторов с использованием системы АмиКард. В данной группе пациентов каждая процедура завершалась  имплантацией кардиомонитора Reveal XT (Medtronic, USA). В течение 2 лет проводилось наблюдение пациентов с определением частоты развития рецидивов нарушений ритма по данным кардиомониторов и холтеровского мониторирования.</p></sec><sec><title>Результаты</title><p>Результаты. По данным неинвазивного картирования у 43 пациентов выявлено 98 роторных очагов (среднее количество на одного пациента составило 2.33±1.09). Так, 84  ротора левой локализации, из них 23 – в легочных венах и только 14 находились в правом предсердии. За два года наблюдения преимущество подхода аблации роторных очагов дополненной изоляцией легочных вен продемонстрировало статистическую достоверность (79% стабильного синусового ритма) по сравнению с антральной изоляцией легочных вен (58% эффективности).</p></sec><sec><title>Заключение</title><p>Заключение. Проведение неинвазивного картирования и аблации роторных областей с замыканием на  анатомические ориентиры в дополнение к антральной изоляции легочных вен способствует повышению эффективности процедуры и позволяет снизить частоту рецидивов у пациентов с персистирующей формой фибрилляции предсердий. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title> Goal</title><p> Goal. To determine the possibilities of driver or rotor ablation with the pulmonary veins isolation in patients with persistent forms of atrial fibrillation in reducing the frequency of recurrences in comparison with the classical method of antral isolation of the pulmonary veins.</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included 86 patients who were divided into group A of antral isolation of the pulmonary veins and group B, in which the isolation of the pulmonary veins was expanded by ablation of the rotary regions in the left atrium. Group B underwent non-invasive mapping of the rotors using the AmiCard system immediately before ablation. In this group of patients at the end of each procedure we implanted a Reveal XT cardiac monitor (Medtronic, USA).</p></sec><sec><title>Results</title><p>Results. According to non-invasive mapping data, 98 drivers were identified in 43 patients (the average number per patient was 2.33 ± 1.09). Therefore, 84 rotors of the left atrium localization, of which 23 were in the pulmonary veins and only 14 were in the right atrium. Over two years of follow-up, the advantage of the driver ablation approach with pulmonary veins isolation demonstrated statistical significance (79% of stable sinus rhythm) compared with antral isolation of the pulmonary veins (58% efficiency).</p></sec><sec><title>Conclusion</title><p>Conclusion.
