В исследование включено 18 пациентов обоего пола, средний возраст - 42,4 г. Количественное определение общего холестерина (ОХС) и триглицеридов (ТГ) осуществляли унифицированным ферментативным методом, ХС липопротеинов высокой (ЛВП) и низкой (ЛНП) плотности - прямым гомогенным методом. Генотип по позиции rs7412 гена APOE определяли методом полимеразной цепной реакции (ПЦР) «в реальном времени» с использованием примыкающих проб и плавлением продуктов реакции после ПЦР. Частота СД среди 367 пациентов с разными типами гиперлипидемии по данным анализа ДНК составила 4,9%. ИБС была выявлена у 27,8% больных. Внешние признаки заболевания (различные типы ксантом) были выявлены у 22,2% больных. При сравнении исходного липидного профиля пациентов с СД и лиц контрольной группы отмечены достоверно более высокие уровни ОХС, ТГ, ХС ЛНП и ХС не-ЛВП среди пациентов, тогда как уровни ХС ЛВП плазмы у них были значительно ниже, чем в контрольной группе. На фоне гиполипидемической терапии (статин и/ или фибрат) средние уровни общего ХС, ТГ и ХС не-ЛВП снизились примерно в 2 раза от исходного (р&lt;0,002), а ХС ЛНП - в 1,5 раза (р&lt;0,008). Целевой уровень ХС не-ЛВП среди больных высокого сердечно-сосудистого риска (&lt;2,6 ммоль/л) на фоне терапии не был достигнут ни у кого, а высокого (&lt;3,4 ммоль/л) - только у 2-х из 5 больных. Полученные данные показывают, что, несмотря на благоприятные сдвиги липидного профиля, многие больные с СД на текущей терапии остаются недолеченными; следовательно, для повышения эффективности терапии необходимо увеличивать дозу статина (при отсутствии противопоказаний) и/или комбинировать статины с фибратами.</p></abstract><trans-abstract xml:lang="en"><p>&lt;strong&gt;The aim of this work&lt;/strong&gt; was to describe a series of cases of familial dysbetalipoproteinaemia (FD) - a rare recessive disorder of lipid metabolism. The study included 18 patients of both sexes, mean age was 42.4 years. Quantitative determination of total cholesterol (TC) and triglycerides (TG) was carried out by a unified enzymatic method, high density lipoprotein (HDL) and low-density lipoprotein (LDL) - by a direct homogeneous method. The APOE gene rs7412 variant was determined by real-time polymerase chain reaction (PCR) using adjacent samples and by melting reaction products after PCR. The frequency of FD according to DNA analysis among 367 patients with different types of hyperlipidemia was 4.9%. CHD was detected in 27.8% of patients. Different types of xanthomas were detected in 22.2% of patients. When comparing the initial lipid profile of patients with FD and those in the control group, significantly higher levels of TC, TG, state Budget-funded Institution National Medical Research center of cardiology of the Ministry of Health of the Russian Federation LDL-C and non-HDL-C were observed, while plasma HDL-C levels were significantly lower than in the control group. On lipid-lowering therapy (statin and/or fibrate), the average levels of TC, TG and non-HDL cholesterol decreased approximately 2 times from baseline (p&lt;0.002), and LDL decreased 1.5 times (p&lt;0.008). The goal level of non-HDL-C among patients with high cardiovascular risk (&lt;2.6 mmol/l) during therapy was not achieved in anyone, and high risk (&lt;3.4 mmol/l) was achieved only in 2 of 5 patients.
