Выявление лиц с повышенной частотой ранних признаков стенозирующего атеросклероза коронарных артерий с учетом особенностей коморбидной отягощенности у больных ИБС высокого риска с наличием МФА.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Больные среднего возраста методом иерархического анализа категориальных переменных были разделены на 3 кластера согласно клинической манифестации атеросклеротического поражения сердца, головного мозга и артерий нижних конечностей. У больных проводили оценку клинического состояния, ЭКГ, коронароангиографию, эходоплероскопию сонных и артерий нижних конечностей. По показаниям проводили магнитно-резонансную томографию органов грудной клетки и брюшной полости, спирографию, фиброгастродуоденоскопию, УЗИ органов брюшной полости и почек, при необходимости УЗИ малого таза.</p></sec><sec><title>Результаты</title><p>Результаты. Было установлено, что одним из проявлений ИБС был ОКС с гемодинамически значимым стенозом ПКА (76-90%), который развился у 50% женщин с преимущественным поражением 3-го сегмента ПКА, относящиеся к 3-му кластеру обследованных. В структуре гемодинамически значимых изменений ветвей ЛКА преобладали мужчины 1-го кластера с поражением 6-го сегмента ПМЖА, ДВ, 15 сегмента ОА, при этом нестабильная стенокардия установлена у 54,2% из них, ИМ в анамнезе - у 51,1%. Среди мужчин из числа пациентов 1-го кластера, перенесших ОИМ, элевация сегмента ST выявлена у 36,4% обследованных в сочетании с мерцательной аритмией (21,2%), атерокальцинозом аорты и ее ветвей (15,2%). Частота встречаемости пограничного уровня стеноза (50-75%) ветвей ЛКА показала преобладание мужчин 1-го кластера с поражением 6-го сегмента ПМЖА, с вовлечением ДВ, а также 15 сегмента ОА, сопровождавшиеся клиникой стабильной стенокардии.</p></sec><sec><title>Выводы</title><p>Выводы. Характер поражения коронарных сосудов у мужчин и женщин средней возрастной категории выявил лиц с повышенной частотой признаков стенозирующего ремоделирования с учетом клинической симптоматики и особенностей сосудистой коморбидной отягощенности у больных ИБС высокого риска с наличием МФА.</p></sec></abstract><trans-abstract xml:lang="en"><p>The aim of the study was to identify individuals with increased frequency of early symptoms of stenosing atherosclerosis of the coronary arteries with consideration of the peculiarities of comorbid abnormalities in CHD patients high risk with the presence of MFA.</p><sec><title>Materials and methods</title><p>Materials and methods. Patients of middle age were divided into 3 clusters by the method of hierarchical analysis of categorical variables according to the clinical manifestation of atherosclerotic lesions of the heart, brain and arteries of the lower extremities. Patients evaluated the clinical status, ECG, CAG, echodopplerography carotid and lower limb arteries. According to the indications, magnetic resonance imaging of the chest and abdomen, spirography, fibro-gastroduodenoscopy, ultrasound of the abdominal cavity and kidneys, if necessary, ultrasound of the pelvis were performed.</p></sec><sec><title>Results</title><p>Results. It was found that one of the manifestations of coronary artery disease was ACS with hemodynamically significant PKA stenosis (76-90%), which developed in 50% of women with primary lesion of the 3rd segment of PKA, belonging to the 3rd cluster of the examined. The structure of hemodynamically significant changes in LCA branches was dominated by men of the 1st cluster with lesions of the 6th segment of PHA, DV, 15 segment of OA, while unstable angina was established in 54,2% of them, in the anamnesis - in 51,1%. Frequency of the level of the border of stenosis (50-75%) branches LKA showed a male predominance of the 1-St cluster with a lesion of the 6th segment PMRA involving DV and 15 of the OA segment, coupled with clinic stable angina.</p></sec><sec><title>Conclusion</title><p>Conclusion.
