В исследование были включены 70 больных хронической сердечной недостаточностью (ХСН) с сохранной (группа I) и сниженной (группа II) фракцией выброса (ФВ) левого желудочка (ЛЖ). Помимо стандартной трансторакальной эхокардиографии всем больным проводилась оценка деформации ЛЖ и левого предсердия, а также ротационных свойств ЛЖ с использованием технологии недопплеровского изображения миокарда в двумерном и трехмерном режимах.</p></sec><sec><title>Результаты</title><p>Результаты. Исследования свидетельствуют о том, что у больных ХСН деформация ЛЖ и левого предсердия, а также ротационные свойства, скручивание и раскручивание ЛЖ по данным двух методов были ниже по сравнению с контрольной группой. В обеих группах отмечалось достоверное увеличение размера и объема ЛП, а также снижение деформации ЛП в разные фазы его деятельности и глобальной продольной деформации ЛП. Значения глобальной продольной деформации ЛП у больных I и II группы (-6,68±3,0% и -6,63±3,3%, соответственно) были достоверно ниже по сравнению с КГ (-9,3±7,3%) (р&lt;0,001). Была выявлена достоверная корреляционная связь деформации ЛП с ФВ ЛЖ у больных I группы (r=-0,39; р&lt;0,001) и у больных II группы (r=0,35; р&lt;0,001).</p></sec><sec><title>Выводы</title><p>Выводы. При оценке деформации ЛЖ по данным двух технологий, трехмерный режим продемонстрировал более низкие показатели деформации миокарда по сравнению с двумерным режимом, что, возможно, связано с более полной оценкой деформационных свойств миокарда ЛЖ. Также снижение глобальной продольной деформации ЛП может быть ранним предиктором диагностики ХСН.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective. To study the peculiarities of deformation, rotation properties, twisting and untwisting of the left ventricular(LV), evaluation of the function of left atrium in patients with CHF with preserved and reduced ejection fraction of the left ventricular according to Echocardiography technology Non Doppler imaging in two and three-dimensional regimes (2D and 3D Speckle tracking imaging).</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included 70 patients with chronic heart failure (CHF) with preserved (Group I) and reduced (Group II) ejection fraction (EF) of the left ventricular (LV). In addition to the standard Transthoracic echocardiography, all patients were assessed the deformation of the left ventricle (LV) and left atrium (LA) as well as the rotational properties of the LV using the technology of myocardial image in two and threedimensional regimen.</p></sec><sec><title>Results</title><p>Results. Studies show that patients CHF deformation of the left ventricular (LV) and left atrium (LA), and the rotational properties, twisting and unwinding of the (LV) according to the two methods were lower than the control group (CG). In both groups there was a reliable increase the size and volume of LA, as well as a decrease the deformation of the LA in it phases and global longitudinal deformation. The values of the global longitudinal deformation of the LA in patients group I and II (-6,68±3.0% and-6,63±3.3%, respectively) were reliably lower compared to CG (-9.3±7.3%) (p &lt;0.001). A good correlation was found between LV deformation and LV EF in patients group I (r = -0.39) ( p &lt;0.001) and in group II (r = 0.35)(p &lt;0.001).</p></sec><sec><title>Conclusions</title><p>Conclusions. When estimating the deformation of the left ventricular according to the data of two technologies, the threedimensional mode demonstrated lower myocardial deformation in comparison with the three dimensional regime, which may be associated with a fuller assessment of the deformation of the left ventricular myocardium.
