Основную группу представили 78 пациентов (ОГ), которым выполнялось динамическое наблюдение за агрегацией тромбоцитов (на агрегометре Multiplate) на стационарном и амбулаторном этапах наблюдения (7 сутки, 1 мес., 6 мес., 12 мес., 18 мес.) для выявления первичной и вторичной резистентности к антиагрегантам с последующей коррекцией антиагрегантной терапии. Контрольную группу составили 53 пациента со стандартным лечением антиагрегантами без контроля агрегации тромбоцитов. Всем пациентам выполнялись: общий анализ крови, биохимический анализ крови с определением уровня TnI, С-реактивного белка, миелопероксидазы, фактора Виллебранда, фибриногена; тест генерации тромбина, также ЭКГ, ЭхоКГ, ХМ-ЭКГ, коронароангиография.</p></sec><sec><title>Результаты</title><p>Результаты: по результатам нашего исследования выявлено снижение чувствительности к клопидогрелу у 24,4% пациентов с НС и стентированием коронарных артерий, из них к оригинальному клопидогрелу - у 7,7% пациентов, к дженериескому клопидогрелу - у 16,7% лиц. Снижение чувствительности к ацетилсалициловой кислоте выявлено у 17,9% лиц с НС. Наличие лабораторной резистентности к АСК и/или клопидогрелу увеличивает относительный риск развития повторных сердечно-сосудистых событий более, чем в 4 раза [ДИ 2,9; 5,6] за 18 месяцев наблюдения, в сравнении с группой лиц без признаков резистентности к антиагрегантам.</p></sec><sec><title>Заключение</title><p>Заключение: выполнение агрегатограммы пациентам с нестабильной стенокардией и стентированием коронарных артерий позволяет своевременно выявлять пациентов с высокой остаточной реактивностью тромбоцитов, выполнять коррекцию антиагрегантной терапии и уменьшить количество повторных кардиоваскулярных событий у пациентов с признаками лабораторной резистентности к антиагрегантам.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective: the aim of the study was evaluation of the clinical significance of high residual platelet reactivity in patients with unstable angina and coronary arteries stenting in the inpatient and outpatient monitoring and development of optimal ways of correction of this condition.</p></sec><sec><title>Material and Methods</title><p>Material and Methods: the study included 131 patients with unstable angina after coronary stenting. The main group represented 78 patients (UA) underwent dynamic observation of platelet aggregation (Multiplate aggregometer) on the inpatient and outpatient observations (7 day, 1 month, 6 months, 12 months, 18 months) to identify primary and secondary resistance to antiplatelet agents with subsequent correction of antiplatelet therapy. The control group consisted of 53 patients with standard therapy with antiplatelet agents without control of platelet aggregation. All patients were performed: general blood analysis, biochemical blood analysis with determination of the level of TnI, C-reactive protein, myeloperoxidase, von Willebrand factor, and fibrinogen; the test of thrombin generation, also ECG, EchoCG, 24-hour ECG monitoring, coronary angiography. The Republican Scientific and Practical Center Cardiology, Minsk, Belarus</p></sec><sec><title>Results</title><p>Results: the results of our study showed a reduction in sensitivity to clopidogrel at 24,4% of patients with UA and stenting of the coronary arteries, of which the original clopidogrel in 7,7% of patients, generic clopidogrel 16,7% pts. Reduced sensitivity to acetylsalicylic acid was detected in 17,9% of individuals with UA. Laboratory resistance to ASA and/or clopidogrel increased the relative risk of recurrent cardiovascular events more than 4 times [CI 2,9; 5,6] during the 18 months of observation in comparison with a group of pts with no signs of resistance to antiplatelet agents.</p></sec><sec><title>Conclusions</title><p>Conclusions: execution of aggregatogram to patients with unstable angina and stenting of the coronary arteries allows to identify patients with high residual platelet reactivity, to correct of antiplatelet therapy and to reduce the number of repeat cardiovascular events in patients with signs of laboratory resistance to antiplatelet agents.</p></sec></trans-abstract><kwd-group xml:lang="ru"><kwd>нестабильная стенокардия</kwd><kwd>резистентность к антиагрегантам</kwd><kwd>агрегатограмма</kwd><kwd>стентирование коронарных артерий</kwd></kwd-group><kwd-group xml:lang="en"><kwd>unstable angina</kwd><kwd>resistance to antiplatelet agents</kwd><kwd>agregatogramm</kwd><kwd>stenting of coronary arteries</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Воробьева И.И., Рыжкова Е.В., Васильева Е.Ю., Шпектор А.В.
